How Do I Become A New Patient?

NEWBORNS

Once the physician discharges your newborn from the hospital, call us to set up your first appointment. We will email you the new patient paperwork to be completed prior to arrival. Our appointment text message will have a link for you to verify your demographics and ask you to start a medical questionnaire.

We ask that you arrive 15 minutes prior to your appointment time for every appointment. That gives us time to complete the check-in process and make necessary updates to your child’s account.

Please bring your completed forms, along with your insurance card(s) and your driver’s license. We do require you to provide your child’s insurance card prior to every visit. If you haven’t already done so, please verify with your insurance company to see if we are in-network and participate with your policy to avoid any unexpected bills.

NEW PATIENTS

Prior to scheduling your child’s first appointment, we require the immunization history and previous medical records to be sent, either via fax (913-894-5522), mail or email to frontoffice@ppadocs.com. Once these have been received, we will call you to schedule your first appointment. Verify with your insurance company before your appointment to see if we are in-network and participate with your policy.

Our Goal Is To Provide The Best Medical Care

For Newborns

When transferring a child to a new pediatric practice, it's essential to gather and organize all medical records, including vaccination records and any relevant test results. Communication with both the previous and new pediatric practice ensures a smooth transition of care, providing necessary information about the child's medical history and ongoing treatments.

Bringing a newborn to a new pediatric practice can be a prudent decision to ensure comprehensive care and support for your child's lifelong health. It allows for establishing a rapport with the healthcare providers early on, fostering a sense of familiarity and trust crucial for ongoing care.

To do this successfully, we must be able to review all of your medical records from all providers that you have seen. It is our policy to receive your previous medical records before we can assume care and schedule an appointment. If you have not already done so, please sign a release of records form and provide it to your children's previous medical provider.

For New Families Transferring To Us

After Transferring Records

Once the records are received, we will review them to determine if they are complete or sufficient for us to begin caring for your children. Unfortunately, we cannot treat or provide any care for your children until that time.

Please be aware that under federal (HIPAA) and state laws, your previous medical provider may charge a "reasonable, cost-based fee" to copy your children's medical records. This cost is your responsibility. This fee may be waived if records are sent directly to our office.
